Forms for Cremation

Completed forms should be returned to the Crematorium at least 2 working days prior to the date of the service, together with a Form 14 (certificate of death registration) which will be issued by a Registrar.

If you complete form A1, A2 or A3 we also require a Service Details Form to be completed. This provides us with all the supplementary information we need to tailor the service to meet your specific individual requirements.

Our media partner, Wesley Media, has an extensive music library and can source most music that has been made commercially available for inclusion in your service. Please click here to view the music listings. If your choice of music is not included in the list, please make your requirements known to your Funeral Director who can organise this on your behalf.

Our Ceremony Hall has the flexibility to adapt the seating arrangements to suit small, intimate services through to large attendances where our overflow Waiting Room streams live images and audio of the service to mourners.

The following Application Forms are for Health Body or Local Authority Use only:

A4 – Application for Cremation of Pregnancy Loss(es) on or before 24 weeks (For use by Health Body only)

A5 – Application for Cremation of an Adult of Child (For use by Local Authority only)

A6 – Application for Cremation of body parts after post-mortem exam or a whole body or body parts following an anatomical exam (For use by person authorised by the doctor, pathologist or anatomy teacher who has completed Form M or Form N)

A7 – Application for Cremation of body parts following anatomical exam where the deceased died before 14 February 1988 (For use by person authorised by anatomy teacher who has completed Form N)

You can return forms by email or in person. Please make sure we have forms at least two working days before the service so we can review them.
